Application for Marriage License

REQUIREMENTS IN THE APPLICATION FOR MARRIAGE LICENSE:
1. Birth Certificate
2. CENOMAR (Cert. of No Marriage) from N.S.O.
3. Appearance of both parents (for ages 18-24 yrs old)
4. Certificate of Pre-Marriage Certificate (from Rural Health Center)
5. Certificate of Pre-Marriage Counseling from the DSWD (for ages 18-24 yrs. Old)
6. Legal Capacity (if citizen of foreign countries)

Payment of necessary fees as per Article 30, Section 123 of the Municipal Ordinance No. 2004-02  of Balungao, Pangasinan, Series of 2004.
